Conference Fees:
  $ 385.00 Post Marked through January 15, 2001
  $ 425.00 After January 15, 2001

Confirmation:
When your registration has been processed you will receive a confirmation letter by mail. However, on arrival at the hotel you are requested to please sign in at the conference registration desk where you will receive your badge, program, information and other materials.

Cancellation:
All cancellation requests must be made in writing. No phone cancellations will be honored. A full refund minus a $50.00 processing fee will be made for each request postmarked by February 1, 2001. No refunds will be made for requests postmarked after February 1, 2001. All refunds will be issued after the conclusion of the conference. Please send your written request to Pro-Cultura, Inc., 301 Old Sleepy Hollow RD., Pleasantville, NY 10570.

Continuing Education Credits:

CME and CE credits for physicians and nurses will be available. For more information call Pro-Cultura, Inc. at (914) 741-2781 or register on-site at the conference for an additional fee of: $50:00 for CME and $30.00 for CE.

Certificate of Attendance: Will be available for a fee of $15.00. You may register and pay for a certificate of attendance in advance but you must pick up a sign in form on-site.

All conference events will be held at the Roosevelt Hotel, New York City, Madison Avenue at 45th., New York, NY 10017. Tel: (212) 885-6071.

Hotel Accommodations:
The Roosevelt Hotel, New York City, Madison Avenue at 45th., New York, NY 10017,is located in midtown Manhattan two blocks from Grand Central Station. We have obtained a discount rate of $190.00, single or double occupancy (plus tax) for all conference attendees making arrangements prior to February 6, 2001. The conference rate is available for a limited number of rooms at the Roosevelt, so please reserve your room early by calling the Hotel at: (800) 223-5652 and mention that you are with The Asian Therapies for Cancer Conference.

Scholarships:
Pro-Cultura, Inc. is committed to helping people from diverse backgrounds and income levels attend the conference. You can help Pro-Cultura, Inc., assist others attend our conference by including an optional donation to our Conference Scholarship program with your registration.

A limited number of scholarships are available. Please submit a letter with your request to Scholarship Coordinator, Pro-Cultura, Inc., 301 Old Sleepy Hollow Rd, Pleasantville, NY 10570

No phone inquiries will be accepted. Please do not use the regular registration form to make the request.

Volunteers/ Work Study Positions:
